I'd set mine up exactly as 2Crosser mentioned. That said, both the POG and Phaser will sound good in front of the amp. It's really a matter of preference so make sure you try both ways.

Part of what you're hearing there is two guitar tracks, but with that in mind a a little compression and reverb and a tele/strat should be all you need. Keep the bass fairly low and turn the gain up...

Could be anything from a bad tube, to a bad speaker cable to a blown speaker. Try a different speaker cable. Test the amp with a different cab and the cab with a different amp.
