How to wire an AEQ SSR acoustic preamp to the Jack plate (1/4" and XLR)

Does anyone know where I could find the wiring details or wiring diagram for the jack plate of an Ibanez acoustic guitar with an AEQ SSR preamp? This particular preamp has a Red, White, Yellow, and Unwrapped wire, and needs to be wired to a 1/4" and an XLR jack.

I did a little digging, and this is the spec/replacement parts page for my exact guitar:

If this helps, it looks as though it uses the Output jack plate part number 5AJK05D. Does anyone know how this is wired from the factory? If anyone has a couple good pictures from different angles of the wiring to a similar 1/4"/XLR Jack Plate coming from the AEQ SSR preamp, that would be FANTASTIC.

Any help is GREATLY appreciated!
