This isn't a matter of me finding a guitar for myself - more out of curiosity, because it seems like these features don't go together very often.

Here are models I know of that fit all four criteria:

Some Gibson SGs (Standard or Custom w/Maestro, Deluxe, Robby Krieger and Jeff Tweedy sigs, Diablo trem)

Firebird VII (Gibson and Epiphone)

The Gretsch G5445T has all of these components, the body is chambered though, so I'm not sure if that fits your criteria fully.

Not only Deans, but both of Dean Zelinsky's subsequent companies- DBZ and Dean Zelinsky's Private Label- have models that meet those criteria. My fave is the DBZ Imprial w/Bigsby.

Reverend also has several models it ting the description.

Fender - the Strat, Tele, Jazzmaster, Jaguar, etc are all bolt-on and any others I've seen were bolt-on as well. I didn't even know they made any set-neck models.

Ibanez - Almost anything Ibanez makes that comes with two cutaways is bolt-on (RG series, S series, XG/XF series, etc). I only know of one double-cutaway Ibanez design with a set neck, and that's the Artist - which doesn't come with vibrato.

Schecter - Ooo! I see they have a few Blackjack FR models - good find!

ESP/LTD - Their site's taking forever to load (my WiFi is garbage), but I see there's a Horizon and a Jeff Hanneman sig model with two cutaways, a Floyd Rose and neck-through construction. Woo hoo!

Jackson - Something weird's going on, as their website only shows two guitar models, and they're both bolt-on.

Charvel - I'm seeing a lot of bolt-on necks with Floyds, and a lot of set-neck guitars coming with fixed bridges, though I might have missed something. (I haven't looked at Charvel too much, to be honest).

Kramer - A lot of bolt-ons, but the SM-1 seems to fit the bill.

Dean - I'm not seeing any vibrato guitars without bolt-on necks.

Based on the manufacturers you suggested, it looks like what I'm thinking of isn't exactly all that common, but you can find it every now and again from the manufacturers targeting metal players.

I like 'em, too! I've had kinda bad luck with the Gretsch Electromatic line in the past, unfortunately, otherwise I'd probably be snagging one of those in gold. They are sharp looking, and seem to stand out from the rest of the Electromatic line for me. I just missed out on a used one for $400, I've been kicking myself for it.

The entire DCR line- each model limited to 100 guitars- are all set necks w/vibrato, as are most of the Modifier series, the 550F, the Vendetta EMG, and a scattering of others.

Aftermarket bigsby on a Hamer? Those are very comfortable to play.
I also own a Gibson Firebird and it's my favorite guitar (haven't tried the Epiphone).
Epiphones might be your best bet, I've tried the doublecut Gretschs and I couldn't get up to the higher frets very easily.
The PRS SE series are also very comfortable and have good tone.
A lot of this depends on price range.

I know this doesn't help, but..."my own thing." Influences include Gang of Four, Neil Young, Joy Division, and My Bloody Valentine.

Stylistically, I use a lot of chords, I like to be able to bend while I strum, and I tend to make heavy use of a capo - sometimes as far as the twelfth fret. I'm not a big fan of bolt-on necks, just because they've never felt as comfortable to me as set neck guitars. Even a Chinese Epiphone G400 feels more comfortable to me at the neck joint than, say, an American Strat.

At the moment, I'm playing a Gibson SG Studio '70s Tribute with a Stetsbar bridge installed. Can't get enough of the minihumbuckers! That all being said...I just want to keep an eye open for other instruments with the features I've taken a liking to, and I find that I have a hard time finding them in local shops. Bleh. I'm near Sacramento, CA, and price range...price range is whatever I can afford when I'm ready to make another purchase. Could be $500 right now, could be $1000 after a few months of saving up, whatever. It depends on what I'm saving up for.
