Song for passed on mom

So I'm trying to write an acoustic piece for her funeral in a couple weeks and need some help, here are the lyrics and chords I've come up with so far. Now I want this song to be as perfect and from MY heart, obviously, but I really need the help. This isn't going public (performing) so no worries if you want to add/change lyrics. I would greatly appreciate it as long as you understand the direction of the piece, I have no objections of the sort. I'm trying to make it generic in terms of lyrics. Not necessarily about a mother in particular because this is for a friend's mother and he asked me to write something. Thanks to everyone that helps with this, it is very important.

All of my edits here.
And UG screwed up the chords you had. My only chord edits are C over used in the second line though.

For the lyrics it might be good to turn it into a narrative or something like that.
Here's my go at it:

And she had dark/light (colour) eyes that would sparkle
Like my life that she made bright
And her voice so soothing and simple
She was a gift to the whole world round
And I know somewhere in up heaven
She's putting a smile on a downturned face
And I hope that she's knows from this evening
That I'll get on ok

I know I've changed alot but it's just a suggestion, and I did keep your basic content. Oh and I hope your christian, cos it doesn't really work otherwise.
