It's not an open world game, but City-17 from Half-Life 2 definitely had some Orwellian themes to it.

Half-Life 2 has City 17 which is basically the city in 1984. Not really an open-world game though because you can't backtrack or go wherever you want from the beginning.

how 'bout the opposite? the game starts with the main character waking up on a cot in a barracks surrounded by similar people. you find out the barracks are in an underground bunker complex. the group of people surrounding you (and yourself), you eventually discover, are the people being selected to be the next world leaders. you have to do whatever you can, by participating in combat drills, puzzles, logic games, to beat everyone else and become the supreme leader.

that's game 1. the thing is, they don't tell you all that information. you simply find out more and more as the game develops. eventually, you're getting sent out to the real world on missions and tests and such.

Closest thing I can think of is Deus Ex Machina: Human Revolution. But it doesn't quite fit your criteria exactly.
