When I play and hold a single note on any of the top 3 strings (G/B/e) the note isn't constantly smooth, it sounds as if I have a phaser running (quite fast) and there is barely any sustain.

I haven't replaced the strings in a while, but is this more likely due to fret wear? The frets are visibly worn down but I have no experience of when to replace frets.

Any help would be great, thanks.

Try lowering your pickups. Sometimes if they're too high the pull of the magnets on the strings produces such effects

Thanks, I'll try that. It's only just begun to happen recently though and I've had the guitar for a couple of years now. Is it normal for the pickups to shift in height over time?

Uh :-o
The only thing that can happen to pick-ups is that over decades they could loose their power.
Check screws I once had a couple of screws resonating to few notes.
