the ( / )s and ( \ )s in this seem to have something to do with the ( BAR ) at the top not sure on any of these
oh and the ( f ) before the bends dunno that either

fb= fulbend, and the / and \'s means that you should slide to the note writen after the / or \.

-21///\\ = I think it means that you should use your whammybar. First, ///, press it up to get a higher pitch when on the 21st fret, and then \\ press the whammy down again to get back to the 21st fret tone.

It basically means you're not settled on a note before sliding.
When you see 12/15 it is usually the case that both notes are clear, whereas in this case you might start sliding from the 12 but it will be what is described as a grace note where you're sliding away from it as soon as the string is plucked.
General rule when you see slides without a start is to go from two or three frets away - either higher or lower on the neck. Logically the /15 would be from somewhere around the 12 or 13th fret.
