What would be a good bass string gauge for a really low tuning, like maybe drop A# or drop B?

Seriously if you're going to go that low get a 5 string bass.
Just check the gauge on the strings to see which would be more appropriate, ask the dude in the shop if you're unsure.

I think .135 is the standard gauge for a low B on a fiver. Would that be suitable or would you be tuning above the B as well?

Yeah I was thinking nothing lower than a B. So B maximum i guess
