As you can see in the image below, the middle locking nut does not fit all the way don in and is raised up. What could be causing this and is it anything to be concerned with?

Is there something blocking it? or something blocking the screw from reaching the bottom?
I'm looking at mine now and it looks like the only thing that could keep it raised up is something blocking the nut or the screw or your strings are slightly higher.

Also, it sounds unlikely but check to see if the nut has bent or something similar, although I can't see that happening to a metal nut like that.

is the locking nut's screw stripped?

take it off and look for what's in the way, or for crossed threads on the screws.
