In that guitar guide, under number 2 "shapes", it has 5 fingering diagrams. However, the second diagram covers 5 frets. What's meant to be the fingering for this, considering I only have 4 fingers? Do I always use the first finger on the first fret and stretch out for my pinkie to reach the 5th? Or do I have to shift my entire hand up/down one fret?

The way I see it, the fingering should be a) whatever is most comfortable for you and b) whatever is most appropriate for where you're going next.
Practice both ways of fingering it, see which works best for you. Also practice switching between the various shapes so you learn the whole pentatonic scale all the way up and down the fretboard.
