A short one: which adapter should I choose if I want to connect my headphones to my guitar combo via its headphones output? Minijack to 1/4, of course, but mono or stereo?

Most amps are mono but a few hybrid modelers have a stereo headphone out.

Pretty sure that all outputs designed to work with headphones are stereo wrt the jack. Is doesn't matter if the signal is stereo. If the connector wasn't stereo then you would have a dead channel in one ear. So the answer is... stereo.

You'd think so, but I've had multiple amps that had a mono headphone out. It doesn't make any sense but it happens.

Mono as in mono jack, or split mono to a stereo jack?

Mono jack! Basically a crime against humanity.

Unless my 1/4" to 1/8" has been borked all these years and I've been wrong the whole time...
