D/F# is usually played: 2X0232 and G/B is played X20003 or X20033

Does the "/" mean it's one chord over the other??

The / signifies a different than normal bass note.

So D/F# is just a D chord, with the F# bass note. G/B is just a G, but with a B as the bass note.

^^ Any chord you "make up" automatically "actually exists." The only issue is if there's going to be a quick and easy name for it rather than a long list of adds and enharmonics.
