I'd have to say mine is a tie between Arcarsenal from AtDI's Relationship of Command, and Sgt. Pepper's Lonely Hearts Club Band from The Beatles.

I think the introduction to the Mars Volta's 'De-Loused in the Comatorium' is fvckin' fantastic....I love how "Son et Lumière" goes straight into "Inertiatic ESP".
And well, I must agree with those who've mentioned Tool's "Stinkfist".

Take a Bow is a monster of an opener, agreed.

Around the World is a real dirty rock n' roll opener. The intro is chaos. Clipping, distorted, awful sound quality... but it's awesome.

Planet Telex is a great opening track for The Bends.

Talk to Strangers is a very powerful spoken word piece that opens Saul Williams' self-titled album. Really personifies everything he's about.
And yet, to me, what is this quintessence of dust? Man delights not me: no, nor woman neither... nor women neither.

There are many, many albums which have awesome intro's. Here are a few favourites:

Until Kingdom Come by Kamelot (Expedition)
Inferno [Unleash the Fires] by Symphony X (The Odessey)
A Bruise Upon The Silent Moon by Cradle of Filth (Damnation and a Day)
Mein Hertz Brent by Rammstein (Mutter)
Reise, Reise by Rammstein (Reise, Reise)
The Package by A Perfect Circle (Thirteenth Step)
Black Label by Lamb of God (New American Gospel)
Ruin by Lamb of God (As the Palaces Burn)
Laid to Rest by Lamb of God (Ashes of the Wake)
Walk With Me In Hell by Lamb of God (Sacrament)

Just a small taste of what I listen to, but those are some awesome ones, IMO.

cheers,

Oh, and Dante, Judith is track 4 on Mere de Nom, APC's debute CD - not an opener.
