so does this mean:
put a finger on string E fret 1 pluck the string take the finger off
then put another finger on sting E fret 2 and pluck take it off then do the same for fret 3 and 4??

or does it mean put each finger on string E on the fret it says and then strum all strings??

The former. Though you don't have to remove the fingers. You can leave your index finger on fret 1, put your middle finger on fret 2 and pluck the string.

and then for fret 3 I can leave my index finger and middle finger on and then pluck??

is there anytime when you can strum all strings??

A strum would look like this:

E----0--
B----1--
G----0--
D----2--
A----3--
E-------

That's a C chord.....when numbers are directly above/below each other, they're played together. When they're seperate, they're played individually.
