mid 60s yardbirds: Vox AC30 + Gibson Les Paul
mid-late 60s John Mayall and the Blues Breakers: Vox AC30 + Dallas Rangemaster + Les Paul
late 60s cream sound: Marshall Plexi + Gibson SG/Gibson ES-335
early 70s Derek and the Dominos: Fender Champ + Fender Stratocaster (Brownie)
mid 70s ~ late 70s Solo: Soldano SLO-100 + Fender Stratocaster (Blackie)
80s~90s Solo: Soldano SLO-100 + Fender Stratocaster w/ Lace Sensor (Clapton Sig)
early 00s Solo: Fender Blonde Twin + Marshall JCM800 + Fender Stratocaster (Clapton Sig)
modern cream RU, solo: Fender '57 Tweed Twin-Amp + Fender Stratocaster (Clapton Sig)

there u go. only pedal i listed was the dallas rangemaster. Clapton himself dislikes using pedals. only ones were probably the Rangemaster, and the Vox Clyde Mccoy in the 60s.

He does use a Crybaby and/ or Vox wah too and he used a Tele with the Yardbirds. He used Les Pauls in the early days of Cream and a Firebird on the late Cream stuff and in Blind Faith, where he also used his ES-335 and Fender Dual Showmans.
