I'm looking for a reference that will give me a good list of chords along the entire fretboard. I've been playing for a couple years now and I know plenty of different chords and, because of a musical background, i can improvise them together quite well. I feel that i am at a stop point in my learning, though, because all the chords i know are mostly all found below the 5th fret. I really want to be able to make any chord, anywhere i want all the way up and down the fretboard.

Does anybody know any good book or website that may be able to suit my needs?

its very very basic to learn how to "mount" chords, and once you get the notes, you can trow them wherever you want

or maybe something a bit more simple look around for lessons on "CAGED" method of "transposing" chords

go to cyberfret.com then go to guitar chords on the side menu then go to guitar codex. that will give you scales and chords over a fretboard diagram.
