Watch this first ...

What sort of pedal or setup enables John Petrucci to do this? Can any MIDI pedal be used in such a way as to allow one to switch amplifier channels and/or rack effects/patches at the same time so to give such a huge variety of sounds at the touch of a button? If not, what can one use to do this?

you could look at the carl martin octaswitch or combinator 2 to switch effects/channels without midi, or if your amp and effects are midi enabled, you can use a midi controller to do so

Aha, so if your amp and effects are MIDI enabled then you can use a MIDI controller to switch patches and such? Wouldn't you need something to organize your effects? ie, when you press this button, this effects turn on and these ones turn off. Also are there any floor based pedals that do this or are they all rack mount?

What about something like this?

That would let you connect you effects into the loops on the back and then use a MIDI controller to switch certain effects loops on/off letting you switch all your effects at the touch of a button.
