Actually scream, and they don't just scream like you would normally scream either. They scream using their diaphragm if they do it properly.

all of those are correct sort of. a lot of screamers that aren't as good don't have good volume control, especially with outward screams. I think inwards are generally louder. And a lot of times distortion or just a lot of gain in general is used to increase the volume and impact.

most actually scream, very few use distortion/vocoder effects unless they are going for a very specific sound, and you can always tell because it sounds pretty unnatural.

A lot of them just scream, but it's hard work to get the right technique. Some use distortion.
