When I read something like:

Eb7

Does that mean play an E flat dominant seven chord, or does it mean play an E chord with a flat seventh?

Also:

D-7b5

Does that mean play a D minor dominant seven with a flat fifth, or does it mean play a D with a minor seventh and a flat fifth?

Eb7 means a dominant chord. the - sign means it is a minor chord, meaning a b3 and b7 plus the b5 for the -7b5 chord. Jazz fakebooks are known to be poorly written and printed.

You're pretty close, but technically the A# would be considered a Bb because the chord is an Eb, and the G# would be an Ab, because the fifth (an A) is being flattened, rather then the fourth (G) being raised a half step

wrong. it is not a minor chord. it is a E flat. its still a dominant 7th though
an Ebm7 would be an E flat minor 7th

Dmaj7 or D*deltasymbol*, are major seventh chords: 1 3 5 7.

Dmin7, D-7 are minor seventh chords: 1 b3 5 b7

Dmin7b5 or D*slashedcircle* are half diminished seventh chords: 1 b3 b5 b7

D*circle* are full diminished chords: 1 b3 b5 bb7
