In the trem arm is there supoosed to be a circular washer inside the cylindrical screw piece??

the part that connects the bar to the guitar grab that between your fingers and tighten it this way.

i think this has to do with your problem.
i had the exact same problem with my ibanez.
no do not buy a new one you dont need to.
open up your back plate and you should see this

all you need to do is get a pair of needle nose pliers and hold the screw in place. while holding it in place, all you need to do is turn the whammy bar until it tightens
