Fret hand or pick hand? How many minutes/hours do you practice before it starts? How long have you been playing?

Sounds to me like they're either just over exerted or you're sitting in an ackward position and your hands can't quite get comfortable. Some more info would help.

Actually, depending on the pain, it's the opposite. Pain is your body's way of saying you're doing somthing wrong.

Fingertip pain is normal. Wrist/lower hand pain after extended periods is fairly common. Anything else and you need to examine your technique.

You are probably wrapping your hand around the neck to much or using to much pressure. You don't need to kill the strings into the frets just enough to get the sound.

Ensure your wrist is in line with your forarm and that you are relaxed the whole time. Thumb should be in the middle of the spine and nice and easy does it. If your doing all this post a vid of yourself playing and let us take a look.

Sounds like exertion. When it starts to hurt, take a little break and relax it. Let your hand be limp, shake it out, and stop playing until it goes away. A certain degree of playing through the pain is O.K., and even required, but make sure you don't hurt yourself. Carpal Tunnel and Tendonitis are very real risks to guitarists, and you'll end up not playing for longer then just a few minutes if you overexert yourself.
